Benefits eligibility varies by the number of hours a team member is scheduled to work.
Here is a quick summary:

If you work 72 to 80 hours per pay period or 36 to 40 hours per week:
You’re considered full time for health plan rates. You’re also eligible for dental and vision coverage, 401(k) or 403(b) retirement plan, vacation time, life insurance coverage, sick time, company-provided short-term disability coverage, company-provided long-term disability coverage, parental leave and holidays including a personal holiday.

If you work 60 to 71.99 hours per pay period or 30 to 35.99 hours per week:
You’re considered full time for health plan rates. You’re also eligible for dental and vision coverage, 401(k) or 403(b) retirement plan, vacation time, life insurance coverage, sick time, company-provided short-term disability coverage, company-provided long-term disability coverage and parental leave. You are not eligible for holidays including a personal holiday.

If you work 40 to 59.99 hours per pay period or 20 to 29.99 hours per week:
You’re considered part time for health plan rates. You’re also eligible for dental and vision coverage, 401(k) or 403(b) retirement plan, vacation time and life insurance coverage. You are not eligible for sick time, company-provided short-term disability coverage, company-provided long-term disability coverage, parental leave or holidays including a personal holiday.

If you work 39.99 or fewer scheduled hours per pay period or 19.00 or fewer scheduled hours per week or as needed:
You’re eligible for the 401(k) or 403(b) retirement plan. However, you are not eligible for health plan rates, dental and vision coverage, vacation time, life insurance coverage, sick time, company-provided short-term disability coverage, company-provided long-term disability coverage, parental leave or holidays including a personal holiday.

Health Plan:
Carle Health offers a PPO low-deductible copay plan and an HDHP high-deductible coinsurance plan with a Health Savings Account (HSA). Both plans include prescription drug coverage.

Eligibility:
For full-time team members, who work 30 hours or more per week, Carle Health covers a larger percentage of the premium costs, while team members pay the remaining amount.

For part-time team members, who work between 20 and 29.99 hours per week, Carle Health covers more than half of the premium costs, while team members pay the remaining amount. 

Team members, their spouse/ domestic partner and children/dependents are eligible for coverage.

Effective date:
First date of employment.

Health Savings Accounts begin the first day of the month following employment.

Dental Plan:
Carle Health offers two types of dental plans that cover most types of preventive care. Other dental care and treatment is paid at a percentage.

Eligibility:
For full-time team members, who work 30 hours or more per week, Carle Health covers approximately 80 percent of premium costs, while team members pay the remaining 20 percent.

 For part-time team members, who work between 20 and 29.99 hours per week, Carle Health covers approximately 60 percent of premium costs, while team members pay the remaining 40 percent. 

Team members, their spouse/ domestic partner and children/dependents are eligible for coverage.

Effective date:
First date of employment.

Vision Plan:
Carle Health offers two options for vision care. Basic vision benefits are included in both health plans. A voluntary vision plan covers more services.

Eligibility:
Basic vision is covered under health plans. Coverage is available for team members who work at least 20 hours or more per week.

Team members, their spouse/ domestic partner and children/dependents are eligible for coverage under both plans.

Voluntary vision is for any employed team member who works 20 hours or more per week.

Effective date:
First day of the month following employment.

For example, if you start on July 12, your coverage begins Aug. 1.

Flexible Spending Accounts (FSA):
Carle Health offers a medical flexible spending account for any health insurance plan, limited flexibility spending account for dental and vision expenses and dependent care flexible spending account.

Eligibility:
Any employed team member who works 20 hours or more per week.

You don't need to be enrolled in a Carle Health insurance plan to have a medical flex account.

Effective date:
First day of the month following employment.

For example, if you start on July 12, your coverage begins Aug. 1.

Vacation:
Eligible team members accrue time off each pay period. Vacation time can be rolled over, year after year, up to a set maximum.

Team members can cash in a portion of their vacation time up to a maximum each year.

Eligibility:
Any employed team member who works 20 hours or more per week.

Effective date:
Vacation time can be used after 90 days of employment.

Sick Time:
Eligible team members accrue sick time hours based on actual hours paid up to a maximum.

Eligibility:
All full-time team members who are scheduled to work 30 hours or more are eligible and can use eight hours of their sick time benefits for a personal sick day. 

Effective date:
Sick time can be used after 90 days of employment.

Short-Term Disability and Long-Term Disability:
Carle Health provides company-paid short-term disability and long-term disability at no cost for eligible team members. Team members can also purchase additional voluntary short-term coverage.

Eligibility:
Company-paid short-term and long-term disability – All team members who work 30 hours more per week.

Voluntary short-term disability – Any employed team member who works 20 hours or more per week.

Effective date:
Company-paid short-term and long-term disability – Coverage begins six months after employment.

Voluntary short-term disability – Coverage begins the day of the month following employment, if enrolled.

Parental Leave:
Eligible team members receive one week of paid parental leave to support them during the birth or adoption of a child.

Eligibility:
Parental leave is available for team members who work 30 hours or more per week.

Effective date:
Parental leave can be used after completing one year of employment.

Holidays/Personal Holiday:
Eligible team members get an eight-hour personal holiday to use each calendar year and six, eight-hour paid holidays each year — New Year's Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day and Christmas Day. Holidays are effective first date of employment and the personal holiday can be used after 90 days of employment.

Eligibility:
Team members who work 36 hours or more per week.

Effective date:
First date of employment.

Family Medical Leave Act (FMLA)
As part of the Family Medical Leave Act (FMLA), Carle Health provides unpaid, job-protected leave to eligible team members for various medical, family and military situations.

Military Leave (USERRA)
Carle Health offers military leave to allow eligible team members time to fulfill service in uniformed services as required by law or administrative regulation.

Victim’s Economic Security and Safety Act (VESSA)
Pursuant to the Victim’s Economic Security and Safety Act (VESSA), team members who are victims of domestic or sexual violence or who have family or household members who are victims of such violence are allowed time away from work for approved activities.

Other Types of Leave
Carle Health offers team members time off for bereavement leave and jury duty.

Retirement Plans:
Carle Health offers all team members retirement plan options. Team members working for a nonprofit entity are in a 403(b) plan. Team members working for a for-profit entity are in a 401(k) plan. Team members and Carle Health may contribute to a team member’s retirement plan.

New team members and rehires are automatically enrolled after 30 days of employment at a pre-tax contribution rate of 3% per paycheck.

Eligibility:
Any employed team member regardless of hours worked can contribute to their retirement plan.

Employer match may begin after one year of employment in which they have worked 500 hours or more.

Employer discretionary contributions may begin after two years of employment in which a team member has worked 500 hours or more.

Must be age 21 or older.

Effective date:
Upon enrollment in the plan or at 30 days of employment if no action is taken.

Voluntary Critical Illness Coverage
Critical illness insurance provides extra coverage for diagnoses of certain diseases and conditions.

Eligibility:
Employed team members who work 20 hours or more per week.

Effective date:
First of the month following employment.

Voluntary Accident Coverage
Accident insurance pays cash for specific injuries and events resulting from a covered accident.

Eligibility:
Employed team members who work 20 hours or more per week.

Effective date:
First of the month following employment.

Voluntary Hospital Indemnity Coverage
With hospital indemnity insurance, you’ll receive a fixed daily benefit if you have a covered stay in a hospital intensive care unit that occurs on or after your coverage effective date.

Eligibility:
Employed team members who work 20 hours or more per week.

Effective date:
First of the month following employment.

Company-Paid Life Insurance Coverage
Basic coverage for full-time team member provides a lump-sum death benefit equal to your annual earnings or flat amount for part-time team members at no cost to team members.

Eligibility:
Employed team members who work 20 hours or more per week.

Effective date:
First of the month following employment.

Voluntary Supplemental/ Dependent Life Insurance Coverage
Having the right amount of life insurance can help protect your loved ones and provide for their future.

Eligibility:
Employed team members who work 20 hours or more per week.

Effective date:
First of the month following employment.

Educational Assistance
Tuition reimbursement allows team members to be paid back for eligible tuition costs after completing their classes.

Amounts and eligibility vary based on role and range from $2,000 to $5,250 per year for full-time team members.

Carle Health also partners with area and online colleges, universities and community colleges to provide additional grants, scholarships and discounts.

Methodist College
As a Carle Health affiliate, Methodist College students can experience a wide range of clinical rotations, practicums and internships at eight partner hospitals and over 100 clinic locations. Upon graduation, students have a direct pathway to a career at Carle Health, with guaranteed job placement for those who meet the qualifications. Carle Health team members also benefit from tuition reimbursement programs.

Eligibility
Team members pursuing a nursing degree or attending Methodist College are eligible to apply for tuition reimbursement upon hire.

Other programs are open to team members who work 20 hour or more per week who have completed six months of service at the time when courses begin.

Employee Assistance Program (EAP)
The Employee Assistance Program (EAP) is a free, confidential assessment, counseling and referral service for team members and their family members. This service supports team members’ mental health, helps them navigate personal, family, parenting and workplace issues, and offers resources for legal and budget concerns.

Eligibility:
Any employed team member regardless of hours worked.

Effective date:
First day of employment.

Adoption Assistance
Carle Health assists and supports team members and their families by assisting with the expenses incurred in the adoption of children.

Eligibility:
Team members working at least 20 hours per week or more are eligible to receive reimbursement for adoption expenses.

Effective date:
One year after first date of employment.
